Spacing Between Teeth

Gaps between teeth can develop when the size of the teeth is too small relative to the size of the tongue and jawbones, when one or more teeth are missing, or due to other factors—such as the mucosal fold between the upper front teeth, known as the labial frenum—that prevent the teeth from aligning perfectly side by side. In such cases, orthodontic treatment can be used to close the gaps.
Spacing can be disadvantageous both in terms of speech and aesthetics. A gap-free dentition is often a prerequisite for an ideal bite and a beautiful smile, although it typically does not cause health problems. In childhood, during the phase of baby teeth and tooth replacement, spacing is considered normal and usually does not require treatment. In other cases, however, treatment may be advisable.
In mixed dentition (when baby and permanent teeth are present simultaneously), gaps between the incisors are often treated only after the eruption of the canines. Once all permanent teeth have emerged, spacing may occur because the jawbone length is relatively greater than the combined width of the teeth. Other causes may include missing tooth buds, tooth extraction, or abnormal size, position, or function of the tongue. A gap between the upper central incisors may be caused by a supernumerary tooth within the bone or a thickened labial frenum. The cause of spacing can be identified with an X-ray.
If treatment is required, a fixed orthodontic appliance is usually necessary. The gap can be closed by moving the natural teeth or preserved for prosthetic solutions such as a bridge or implant. The exact treatment depends on several factors, including the size and location of the gap, and the condition and position of the teeth. In cases of tongue dysfunction, a speech therapist may need to be involved in the treatment. To maintain the results achieved, a retainer is especially important.
