Jawbone Anomalies

Various health issues may arise if, during growth, the lower jaw and the upper jaw—which develops in conjunction with the sinus region—do not grow at the proper pace or in harmony with each other. In such cases, orthodontic treatment alone is not sufficient; the involvement of an oral surgeon becomes necessary.

Orthodontic treatment alone is not always sufficient—some discrepancies require additional procedures. A combination of orthodontics and surgery may be necessary when the underlying issue lies not in tooth positioning, but in abnormal jaw growth or positioning. Before and during adolescence, jaw growth and position can be influenced to some extent using removable appliances. However, in adulthood, as growth significantly slows, this is no longer possible. For optimal aesthetic and functional results, a more invasive procedure may be required to correct bite irregularities.
Depending on the type of anomaly, surgery may involve the upper jaw, lower jaw, or both. Following the treatment plan discussion, a surgical consultation typically takes place where we, together with the oral surgeon, provide detailed information regarding the procedure. Surgery is preceded by either a short or extended orthodontic phase, depending on the case specifics and accompanying tasks. Because the procedure is carried out under general anesthesia, additional preoperative tests (EKG, chest X-ray, lab work) are required—details will be shared during surgical scheduling. For such treatment, precision is key; millimeter-level planning is critical and supported by advanced software and 3D imaging.
Once orthodontic pre-treatment is complete, the surgery date can be scheduled and adjusted to the patient’s preferences based on the surgeon’s availability. Hospital admission typically occurs one day before the surgery, with a post-operative stay of 3–7 days. During the procedure—under general anesthesia—the jawbone(s) are repositioned and stabilized using plates and screws. For surgeries aiming at jaw expansion, the bone is only weakened so the pre-attached appliance can act not just on the teeth, but also on the jaw. Immediately after surgery, surrounding tissues are swollen and mouth opening is limited; discomfort is generally manageable with pain medication.
Recovery typically takes 1–2 weeks and is mainly marked by facial swelling and discoloration. Although patients are not bedridden, certain precautions are necessary: maintaining oral hygiene is essential for healing despite the difficulty of cleaning, and hard foods and contact sports should be avoided for the first 6 weeks. After surgery, orthodontic treatment continues. In some cases, procedures are not completed in a single stage but require multiple sessions—this may be needed if jaw expansion is necessary beforehand or if modifying the chin is desired for improved aesthetics.
This approach may involve multiple surgeries under general anesthesia, with at least 6 months between them. Since even combined surgical-orthodontic treatment cannot guarantee full long-term stability, retaining the results with proper retention methods is essential after treatment
